Customer’s Sales Order date must be within the promo period of July 4, 2017 to October 1, 2017. BONUS REBATE: a. Purchase of 3 (three) different categories in one qualifying kitchen package, receive $300 additional rebate. b. Purchase of 4 (four) different categories in one qualifying kitchen package, receive $400 additional rebate. To qualify for BONUS REBATE, the qualifying categories must be purchased on the same Sales Receipt, and must be NEW orders placed within the PROMO PERIOD of July 4, 2017 to October 1, 2017. T E R M S A N D L I M I TAT I O N S 1. Original or copy of rebate form and supporting documents must be postmarked no later than March 31, 2018. No late submissions will be accepted. Consumer shall be solely responsible for lost, late or misdirected mail. 2. Please allow eight to ten weeks to receive your rebate check. 3. Products must be purchased from participating dealers in the provinces covered by Monde Home Products Ltd., namely, British Columbia, Alberta, Saskatchewan, Manitoba, Yukon, and Northwest Territories. 4. Customer sales receipt must be dated within the promo period of July 4, 2017 - October 1, 2017. Purchases made before or after these dates will not be eligible. Purchase date is determined by invoice date or down payment date. 5. There is no limit as to number of units purchased per customer to qualify for the rebate. 6. Rebates will be issued for qualifying models only. 7. Any mis-representation or fraudulent information disqualifies this claim and may give rise to criminal or civil prosecution. 8. Bertazzoni distributors, resellers and their employees, Bertazzoni employees and internal Bertazzoni orders are ineligible. 9. Only retail purchases by an end-user are qualified. Not open to single family/multi-family builders or contractors. 10. Rebate will be distributed in the form of a check. Rebate checks are void if not cashed within 60 days of issuance and cannot be reissued. 11. Monde Home Products is not responsible for claims received late, lost, misdirected, mutilated, stolen or with postage due. Please keep copies of all materials submitted. 12. Monde Home Products reserves the right to request additional information regarding this claim, and the right to confirm identification. All information submitted with this claim becomes the property of Monde Home Products and cannot be returned. No group organization request will be honored and author rights may not be assigned or transferred. 13.
