CLINICAL SKILLS
This is meant to be a student reference for the CNA licensure test, to remind you of the steps to the procedures you have already learned in lab. Some of the steps may not be detailed and it may be assumed that you know the basic procedures by name. This is not meant to be a teaching device. Each of these skills can also be found in the CNA textbook on the pages referenced.
HANDWASHING (pg 205) Supplies: 1-Soap 2-Paper towels 3-Wastebasket. 1. Push your watch and sleeves up your arm 4-5 inches. 2. Stand away from the sink so your clothes do not touch the sink –Do not touch the inside of the sink at any time. 3. Turn on water and adjust temp. 4. Keep hands below wrists. 5. Apply soap, interlace fingers and lather 10-15 seconds 6. Rub fingertips in palms to clean fingernails 7. Rinse well. 8. Dry hands starting at fingertips, “Candy-cane”, throw away paper towel 9. With a new paper towel, turn off faucet and then throw away paper towel. Hand washing and cleaning underneath nails w/ and orange stick and filing nails smooth should be done first thing of the day. Subsequent washings will not include orange stick or nail file.
Testing Checklist: Does candidate: work up lather cleansing front and back of hands, between fingers, around cuticles, under nails & one hands-width up the wrist? Provide cleansing friction for minimum 10 seconds? Keep hands lower than wrists? Dry hands w/ paper towel and limit contact of towel to cleansed skin surfaces? Turn off water with paper towel and dispose of paper towel? Complete task without contaminating hands, such as against the sink?
BEDPAN (pg 358) 1. Knock, greet resident by name and introduce self and title. Explain intended procedure and ask permission. 2. Excuse self to wash hands and gather supplies. Give call button. 3. Simulate hand wash and gather supplies Supplies:
1-gloves 2-bedpan 3-T.P. 4-Chux pad 5-washcloth 6- Towel (2)
4. Knock, greet resident by name and begin set-up 5. Cover side table with towel and lay supplies down on top. 6. CLOSE CURTAIN FOR PRIVACY 7. Raise bed 8. Gloves 9. Fold top linens down, cover pt. with towel 10. Place chux pad under patient (rolling to side) 11. Place bedpan under patient 12. Raise head of the bed to Fowler’s position, checking bedpan position & pt. comfort 13. Give T.P. and call light and ask them to signal when finished, excuse self to the other side of curtain – IF you leave the room you must re-glove and wash hands, but you are technically staying in the room. 14. Knock or ask permission to re-enter asking if patient is finished, greet resident by name – offer washcloth 15. Lower head of the bed to supine 16. Roll pt & remove bedpan Cover with towel (from table) 17. remove chux 18. Pretty beds, lower beds, call bell, check comfort, open curtain 19. Empty bedpan into toilet and disinfect – replace at bedside table bottom shelf 20. Remove gloves and wash hands
Testing Checklist: Did candidate provide for comfort? Place protective pad before bedpan? Raise head of bed after placing bedpan? Provide T.P before removing pan? Lower head of bed before removing pan? Empty contents into toilet? Disinfect bedpan and place on bottom shelf of bedside stand? Wash hands before and after? Glove twice, changing in-between?
DRESSING (pg350) 1. Knock, greet resident by name and introduce self and title. Explain intended procedure and ask permission. 2. Excuse self to wash hands and gather supplies. Give call button. 3. Simulate hand wash and gather supplies Supplies:
1- 2 changes of clothing 2- Socks & underwear 3- non-skid footwear 4-Towel 4. Knock, greet resident by name 5. Set up table, cover with towel and lay supplies down 6. CLOSE CURTAIN FOR PRIVACY 7. Offer choices 8. Remove clothing from strong side first, then affected side 9. Dress affected side first, then strong side 10 pretty beds, lower beds, call bell, check comfort, open curtain 11. Testing checklist Does the candidate: Remove and dress in the correct order (affected side vs. strong side)? Gather up sleeve to ease pulling over affected arm? Move body and extremities gently? Adjust clothing for comfort, neatness and alignment? Place dirty gown in designated hamper Wash hands before and after?
FEEDING (pg 421) 1. Knock, greet resident by name and introduce self and title. Explain intended procedure and ask permission. 2. Excuse self to wash hands and gather supplies. Give call button. 3. Simulate hand wash and gather supplies Supplies:
1-food tray 2- 2 choices of food, spoon 3- Full glass water (240ml) with straw 4- Shirt protector 5-napkin or facial washcloth 6-towel

Testing checklist Did the candidate offer and assist to wash hands before feeding? Sit at eye level? Apply shirt protect? Offer fluid before food? And then between 3 bites of food? Check to see if they’ve swallowed before offering next bite? Offer encouragement in eating & fluid intake. Converse. Clean and dry face? Clean area. Record intake Wash hands before and after?

Test checklist: Did candidate check comfort level? Control and support the extremity? Use slow, smooth, non-forced movements? Provide 3 repetitions of each exercise?
PERICARE (pg 330) (Catheter care is same) (Wiping 3 strokes, wash/rinse 4” up catheter line) 1. Knock, greet resident by name and introduce self and title. Using Rinse washcloth – (verbalize water temp) Same (3) middle (3) side and top 12. Pat dry 13. Roll patient onto side (facing away from you) keeping covered. 14. Using soapy washcloth Use 3 strokes from cleanest to dirtiest (vag to anus) Cheek-cheek-middle Always using a clean part of the washcloth for each wipe 15. Rinse – same way 16. Pat dry 17 Remove chux and replace with dry chux 18. Cover w/ gown and remove towel. Cover with blanket 19. Pretty beds, lower beds, call bell, check comfort, open curtain 20 gather laundry/ and supplies (don’t touch scrubs) 21. Empty and clean basins and return to proper place. Testing checklist: Did candidate pass over “urinary meatus” with 1st stroke of washcloth? Change spot on washcloth each stroke? Pat dry?
PARTIAL BED/BATH (pg 320) 1. Knock, greet resident by name and introduce self and title. Explain intended procedure and ask permission. 2. Excuse self to wash hands and gather supplies. Give call button. 3. Simulate hand wash and gather supplies Supplies:
1-gloves 2-(2) basins 3- Soap 4- (2) towels, (2) washcloths 5- (1) bath blanket 6- Hospital gown 7- Lotion Verbalize checking water temperature – do not put soap into water!

Place towel lengthwise underneath arm 13 (wash water & rinse) neck, arm, hands – both side – PAT DRY 14. (Wash & rinse) chest, under breasts - PAT DRY 15. Roll pt. away from you –keeping covered. (Wash & rinse) back – PAT DRY 16. Lotion and massage back – pat dry unabsorbed lotion Verbalize check comfort level? 17. Remove and replace hospital gown 18. Pretty beds, lower beds, call bell, check comfort, open curtain 19. Clean up laundry, water into toilet, rinse & sanitize basins and return to bedside stand. 20. Wash hands Testing checklist: Did candidate ensure safe, comfortable water temp? Minimize exposure of patient at all times? Use washcloth w/o soap for washing face? Wipe eye from inside to out, changing to clean area of cloth with each wipe? Leave areas clean and dry? Use “wash mitt” and contain corners? Protect bedding from getting wet by repositioning towel throughout? Apply small amt. soap directly to washcloth? Warm lotion before applying to pt. back? Replace hospital gown with minimal exposure? Wash hands before and after?
BLOOD PRESSURE (pg 459) 1. Knock, greet resident by name and introduce self and title. Explain intended procedure and ask permission. 2. Excuse self to wash hands and gather supplies. Raise head of bed to semi-fowlers, adjust table height as needed – Patient should be able to rest arm comfortably on the side table. 8. Wash ear pieces and diaphragm of stethoscope w/ alcohol 9. Listen for point pulse last heard/felt +30 (fully deflate) Then measure Blood Pressure. 10. Re-wash ear pieces and diaphragm of stethoscope w/ alcohol 10. Testing checklist: Did Candidate verbalize safe water temperature? Remove residue from under nails? Dry hands after soaking? Leave nails smooth w/o jagged edges? Apply lotion? Clean and store equipment leaving work area tidy? Wash hands before and after?
CHANGE AN OCCUPIED BED (pg 298) 1. Knock, greet resident by name and introduce self and title. Explain intended procedure and ask permission. 2. Excuse self to wash hands and gather supplies. Give call button. 3. Simulate hand wash and gather supplies
Supplies: 1- Gloves 2- Fitted sheet & flat sheet 3- Pillow case 4- Towel (to cover table top) 4. Knock, greet resident by name 5. Set up table, cover with towel and lay supplies down 6. CLOSE CURTAIN FOR PRIVACY 7. Adjust bed height as needed 8. Begin by removing upper and lower corners of the fitted sheet and tucking under pt. 9. Place new fitted sheet on corners and tuck underneath patient -do the same with the mattress pad or draw sheet if required 10. Place new flat sheet lengthwise with fold up on patient’s Rt. side 12. Move to opposite side of the bed, carefully roll patient away from you to expose and pull out the dirty linens (rolling to the bottom of the bed) 13 (keeping patient covered) pull the clean linens from under the patient and roll patient back down to supine. –no wrinkles under patient! 14 Place new fitted sheet on corners of bed 15. Reach over patient to the Rt. side and grab the fold of the new flat sheet along with the edge of the old flat sheet. Pull the new flat sheet across the patient (removing the old flat sheet and applying the new flat sheet at the same time) Roll the old flat sheet to the bottom of the bed along with any other dirty linens. Tuck new sheets. Add blanket on top if required. 16 Remove pillow from patient. Remove case and re-case. (Turning new case almost completely inside-out to place on the pillow) Return to beneath patient’s head carefully lifting head. Be careful not to touch uniform. 17. Remove dirty linens from bottom of bed and wrap in towel that was used on the side table. Keep away from your uniform. 17. Pretty beds, --not tucked too tight on toes, no wrinkles underneath patient, 18. Lower bed, call bell, check comfort, open curtain Remove gloves, wash hands, dispose of dirty linens Testing checklist: Did candidate position patient safely on side and non-working side of bed? Roll dirty bottom linens and tuck under pt? Assist patient in turning/rolling? Are sheets secured? Bottom sheet free of wrinkles? Minimize exposure? Allow for foot movement (not too tight?) Replace pillowcase? Wash hands before and after?

Move to opposite side of bed, position patient’s legs so that top leg is bent 90o at knee and hips are aligned. Avoid bony prominences touching (ankles, knees) Pillow #2 – Place flat between knees 11. Position Lt shoulder forward slightly and at 90o to body Pillow #3 – place at chest level tucking slightly to avoid patient rolling onto stomach 12. Position head pillow to provide support and alignment of neck 13. Verbalize comfort level 14. Pretty beds, lower beds, call bell, check comfort, open curtain 15. Wash hands

MEASURING AND RECORDING U-BAG (pg371) 1. Knock, greet resident by name and introduce self and title. Explain intended procedure and ask permission. 2. Excuse self to wash hands and gather supplies. Give call button. 3. Simulate hand wash and gather supplies Supplies: 1-graduated measuring cup 2-gloves 3-paper towels (2) 4-alcohol wipes (2) 4. Put on gloves 5. Knock, greet resident by name- CLOSE CURTAIN FOR PRIVACY 6. Place paper towels on the floor and place graduate on top right beneath the U-bag 7. Verbalize checking that U-bag tubing is unobstructed, not hanging to the floor, and that the U-bag is hanging on an immovable part of the bed. 8. Release clamp and drain required amount from U-bag – do not let the drain touch the graduate. 9. Re-clamp the drain and clean it with alcohol wipe, then return the drain to the “pocket” 10. gather supplies from floor 11. check patient comfort, call bell and excuse self, open curtain 12. Measure amount of urine by placing on a paper towel on the counter and reading at eye level (w/in 50cc of CSO) 13. dispose of urine, rinse graduate, --then remove gloves 14. wash hands 15. Record urine output on I&O sheet w/ time, date, initials Testing checklist: Did candidate avoid touching sides of graduate? Wipe drain w/ alcohol after emptying? Leave bag secured to immovable part of bed, tubing not touching floor? Set graduate on flat surface / eye level to read? Empty into toilet and disinfect graduate? Remove gloves and wash hands before recording? Record output w/in 50cc of CSO? Record correctly on I&O sheet?
ORAL CARE: BRUSH TEETH (pg 309) 1. Knock, greet resident by name and introduce self and title. Explain intended procedure and ask permission. 2. Excuse self to wash hands and gather supplies. Give call button. 3. Simulate hand wash and gather supplies Supplies: 1- gloves 2- kidney basin (marked for oral use) 3- toothbrush 4- toothpaste 5- water glass w/ water and straw if needed 6- washcloth for wiping patient’s mouth 7- shirt protector 8-hand towel (to cover table top) 4. Knock, greet resident by name 5. Set up table, cover with towel and lay supplies down 6. CLOSE CURTAIN FOR PRIVACY 7. Raise the head of the bed to 45o, adjusting the bed height as needed 8. place shirt protector on patient 9. Moisten toothbrush w/ water, apply tooth paste 10. Offer water to drink before beginning. 11. Gently brush teeth – offering patient opportunity to rinse and spit in kidney basin as needed 12. wipe patient’s mouth – leave clean and dry. 13. remove shirt protector 14. reposition pt. to sitting or lying as desired 15. pretty beds, lower beds, call bell, check comfort, open curtain 16. dispose of dirty linens 17. clean and return disinfected kidney basin, toothbrush and toothpaste to bedside 18. remove gloves and wash hands Testing checklist: Did candidate protect patient clothing? Set up table, cover with towel and lay supplies down 6. apply gloves 1-take dentures, toothbrush, toothpaste and excuse self to go scrub dentures – give call button 2- line sink with paper towels 3- remove dentures from cup, place in lid, rinse cup and refill with cool water 4- apply toothpaste to toothbrush and brush dentures, rinse well and place in clean denture cup 5- remove gloves 6-gather supplies and return to room 7. PULSE AND RESPIRATIONS (pg 452, 456) 1. Knock, greet resident by name and introduce self and title. Explain intended procedure and ask permission. 2. Excuse self to wash hands and gather supplies. Pull chair over --position of patient can be lying or sitting. 7. locate radial pulse; count for 1 full minute (must be +/- 4 bpm of CSO’s count) 8. Continue (unnoticed) w/ respirations for 1 full minute (must be +/- 2 breaths of CSO’s count) 9. Pretty beds, lower beds, call bell, check comfort, open curtain 10. Properly record noting any irregularities (strong, weak, regular, irregular) Testing checklist: Did candidate support resident’s forearm while taking pulse? Count heart rate for 1 full minute? Record w/in 4 bpm of CSO? Count respirations for 1 full minute? Record w/in 2 breaths p/min of CSO? Wash hands before and after?
TRANSFER (pg 260) 1. Knock, greet resident by name and introduce self and title. Explain intended procedure and ask permission. 2. Excuse self to wash hands and gather supplies. Give call button. 3. Simulate hand wash and gather supplies Supplies: wheelchair gait belt 4. Knock, greet resident by name 5. place wheel chair parallel/or barely angled to bed, lock wheels, remove foot plates or swing away 6. CLOSE CURTAIN FOR PRIVACY 7. lower bed to lowest position and assist patient to dangling position – verbalize comfort, “not dizzy?” 8. assist to put on non-skid footwear, sweater if needed 9. apply gait belt 10. verbalize procedure to patient and assure understanding 11. properly raise patient to standing position (bracing knees, blocking the feet – counting 1-2-3, patient pushes up on mattress) 12. pivot patient and help to sit in wheelchair, lowering gently 13. check patient’s comfort and body alignment, re-apply footplates or swing into position. remove gait belt 14. Unlock wheelchair, open curtain and move as necessary 15 Wash hands
Testing checklist: Did candidate position wheelchair correctly before sitting patient up? Lock wheelchair before transfer? Remove footplates before transfer? Place non-skid footwear on patient before transfer? Provide support to assist patient to sitting on side of bed? Apply gait belt correctly? Ensure patient’s feet are flat on floor before beginning transfer? Assist in transfer correctly (pivot while bracing pt. legs, holding gait belt)? Maintain own body mechanics? Lower to wheelchair controlled and gentle? Position resident in proper body alignment in wheelchair? Place resident’s feet on footplates? Remove gait belt? Wash hands before and after?
