What are the key Coverage of my:health Medisure Prime? Following are the key benefits that are unique to my:health Medisure Prime Insurance. This is our endeavour to bring to you innovative features. • Pay for your policy based on your location – The healthcare costs differ in different cities across India. We have carefully factored the same while arriving at the premium. – So if you belong to a city where the cost of a particular treatment is lower than say Mumbai, you will have to pay a lower rate of premium (Read on to know which zone you belong to). • In-patient Hospitalisation Expenses If the treatment of a disease, illness or injury is taken and you are hospitalised as an In-patient for a minimum period of 24 hours we will pay the medical expenses incurred by you towards; – Room Rent/Boarding & Nursing – ICU Rent/Boarding & Nursing – Fees of Surgeon, Anaesthetist, Nurses and Specialists – Cost of Operation Theatre, diagnostic tests, medicines, blood, oxygen and cost of prosthetic and other devices or equipment if implanted internally like pacemaker during a Surgical Procedure This also covers any relapse within 45 days from the date of last hospitalisation at the hospital/nursing home where treatment was taken. Occurrence of the same illness after 45 days would be a fresh instance of illness. • Pre-hospitalisation This covers the relevant medical expenses you incur within a period of 60 days before you are admitted to a Hospital/Nursing Home provided that; i. Such medical expenses are incurred for the same condition for which your hospitalisation was required and ii. The In-patient Hospitalisation claim for such hospitalisation is admissible by us. • Post-hospitalisation This covers the relevant medical expenses, up to a period of 90 days, after you are discharged from the Hospital/Nursing Home provided that; i. such medical expenses are incurred for the same condition for which your hospitalisation was required and ii. The In-patient Hospitalisation claim for such Hospitalisation is admissible by us. Thus, ensuring that you don’t have to worry about expenses post-hospitalisation. • Day Care Treatment This covers the medical expenses for a day care procedure or surgery incurred by you for treatment or procedures that requires less than 24 hours of hospitalisation undertaken under general or local anaesthesia which would have otherwise required hospitalisation. There is no static list for day care procedures in the Policy as advances in medical science leads to many more being added continuously. So, whether it’s recommended by a medical practitioner or even if it is a new procedure, you can be rest assured that, we will cover it. However, this cover excludes diagnostic procedures and treatments taken on an Out-patient basis. AL EW REN • Critical illness The policy provides an additional cover equivalent to the Sum Insured (which essentially doubles the Sum Insured) for treatment of following listed critical illness. a) First Diagnosis of the below-mentioned Illnesses: – Cancer (of specific severity) – Kidney Failure (requiring regular dialysis) – Multiple Sclerosis with persisting symptoms – Primary Pulmonary Arterial Hypertension b) Undergoing for the first time of the following surgical procedures – Major Organ/Bone marrow Transplant – Aorta Graft Surgery – Open Chest CABG c) Occurrence for the first time of the following medical events – First Heart Attack (of specific severity) – Stroke (resulting in permanent symptoms) Insured Person(s) diagnosed with a particular Critical Illness listed above during a policy period shall not be entitled to avail the benefits under the same Critical Illness for subsequent renewals. However, he may continue to be covered for the remaining critical illnesses. • Maternity and New Born Baby Cover – The policy provides coverage towards maternity expenses and new born baby cover up to the limit of Sum Insured below. – Maximum of 5% of Sum Insured or `25,000 for a normal delivery and 10% of Sum Insured or `50,000 for a caesarean section or actual cost, whichever is lower. – Coverage is applicable only towards the first two children for Self and lawful Spouse who are covered under a single policy for a continuous period of 48 months. – New born baby will be covered for a period of 90 days from date of birth within the maternity limits. – Pre or Post natal Medical Expenses will be covered within the limit of Sum Insured under this cover however pre-post hospitalisation expenses are excluded under this cover. • Hospitalisation due to an Accident If during the term of your policy, the entire Sum Insured gets used up and there is a second hospitalisation due to an accident, the Sum Insured of your policy is ‘reinstated’ or ‘replenished’ to the extent of claim amount but not exceeding the Sum Insured. This additional amount will be available only ONCE during the 12-month policy period and shall trigger when the basic Sum Insured opted under the policy has been exhausted or is inadequate to cover this second hospitalisation due to accident. • Value added Covers – Donor Expenses We will reimburse Medical expenses incurred towards donor in case of major organ transplant, for the harvesting of the organ provided that: The Organ donor is any person in accordance with the Transplantation of Human Organs Act 1994 (amended) and other applicable laws and rules, The Organ donated is for the use of the Insured Person. Coverage under this section excludes Pre-Post hospitalisation expenses, donor screening, cost of organ or any other medical treatment for the donor consequent on the harvesting. – Hospital Cash: We will pay for Daily Hospital Cash Allowance of `1000 per day from day 4 to day 10 provided hospitalisation exceeds 3 days continuously. – Ambulance Charges: Reimbursement upto `1500 will be provided for expenses incurred towards transportation by ambulance to the Hospital/Nursing Home for treatment of the disease/illness/injury necessitating admission to Hospital/ Nursing Home. – Recovery benefit: We will pay a lump sum amount of `10,000 in case hospitalisation exceeds 10 continuous days. – Expenses on accompanying person: We will pay `500/- per day for maximum period of 7 days towards expenses of accompanying person for the treatment of Insured in Hospital/Nursing home exceeding 3 days. • Other Features – Two year policy option allows you to renew you policy at the end of 24 months v/s the traditional 12 months – FREE Health Check up after every two consecutive renewals once you are above 45 years at the time of the 2nd renewal – Pre-Existing Diseases: Medical expenses arising out of your pre-existing diseases are covered from the 3rd year of your Policy, after you have been continuously insured with us for 24 months – Instant Policy issuance and no medical test if you are below 45 years at the time of policy issuance with no adverse medical history. – Lifetime renewal of Policy What is the HDFC General Insurance Advantage? Your my:health Medisure Prime offers you a host of value added benefits: • Family Floater Option: You can cover up to 4 members of your family – your lawful spouse & 2 dependent children up to the age of 18 years under a single policy and single sum insured. • Family Discount: If you choose to opt for Individual Sum Insured for each of your family members under a single policy instead of a Family Floater option, you are eligible to get a Family Discount of 10% on the total premium payable for covering more than 2 members which include yourself, spouse, dependent children and dependant parents. Family discount will be available for a maximum of 6 persons only. • Cumulative Bonus: The ‘Sum Insured’ of your policy progressively increases by 5% on every renewal, for every claim-free year, subject to a maximum accumulation of 50% of your Sum Insured (If a claim is made, the Cumulative Benefit that has accrued will be reduced by 5%; however, the basic Sum Insured will be maintained at all times). • Income Tax Benefit: You can get tax exemption on the premium paid under section 80D of the Income Tax Act. • Free-Look Period: 15 days from date of receipt of Policy documents, you have the option to cancel the Policy if you are not satisfied with the coverage, terms of the Policy, provided there is no claim. A minimum premium towards medical test charges, stamp duty and proportionate risk premium will be retained by us (If Policy has already commenced). Refund will not be applicable if you have made a claim against the Policy during that period. Who all can be covered under my:health Medisure Prime and what is the age limit for different family members? • Entry age for you (the proposer) is 18 years and you can opt for this policy up to the age of 65 years • Renewal of the Policy is for your lifetime • Children above the age of 3 months and adults below the age of 65 years can be covered in this policy • Children up to the age of 18 years can be covered only if one or both parents are covered • If you are above the age of 45, your policy issuance is subject to Pre-acceptance medical tests and subsequent approval by us What are the Medical Test Requirements? All Individuals up to 45 years (age as on last birthday as at Policy inception) have to fill a declaration on the Proposal Form. If any medical adversity exists, only then will you be required to undergo appropriate medical tests. The age group 46-65 years (age as on last birthday as at Policy inception) will be required to undergo the following pre-acceptance medical tests Scenarios Medical Tests Requirement Age* group 46 - 55 years Medical Examination Report, Electrocardiogram, Serum Triglycerides, Fasting Blood Sugar, Serum Creatinine, Urinalysis, SGOT & SGPT. Age* group 56 - 65 years Medical Examination Report, Electrocardiogram, Complete Blood Count, Lipid Profile, Fasting & Post Prandial Blood Sugar, Serum Creatinine, Urinalysis, SGOT, SGPT and GGT. *Age as on last birthday as at Policy inception date to be considered. Medical tests will be conducted at our list of diagnostic centers and 100% cost for the same will be borne by us. Based on the evidence from the above reports the Company may call for additional reports on a case to case basis to determine the acceptance of a Proposal. 50% Cost (On our pre agreed rates with the network provider) of any additional test shall be borne by the Insurer for accepted proposals. Medical Underwriting Proposers above 45 years of age and those having medical history are subject to Medical Underwriting by the Company. We reserve the right to accept such proposals on standard terms/Decline/Accept with exclusion or Premium loading (up to maximum of 100% on basic Premium). These loadings are applied from Commencement Date of the Policy including subsequent renewal(s) with us. What is the range of Sum Insured under my:health Medisure Prime? `3 lac to `10 lacs in multiples of 1 lakh. What does my:health Medisure Prime not cover? • All pre-existing diseases/illness/injury/conditions as defined in the Policy, until 24 months of continuous covers have elapsed since inception of the first Policy with us. • Any disease contracted and/or medical expenses incurred in respect of any disease/illness by the Insured/Insured Person during the first 30 days from the commencement date of the Policy except in case of accidental injuries. This exclusion doesn’t apply for Insured/Insured Person having any health insurance indemnity policy in India atleast for 1 year prior to taking this Policy as well as for subsequent renewals with the Company without a break. • All expenses along with their complications on treatment towards Cataract, Hysterectomy for Menorrhagia or Fibromyoma, Knee Replacement Surgery (other than caused by an accident), Arthritis, Rheumatism, Joint Replacement Surgery (other than caused by accident), Prolapse of Intervertibral discs (other than caused by accident), Varicose Veins and Varicose Ulcers, Hernia, Stones in the urinary uro-genital and biliary systems, Benign Prostate Hypertrophy, Hydrocele, Congenital internal anomoly, Fistula in anus, Piles, Pilonidal sinus, Chronic Suppurative Otitis Media (CSOM), Deviated Nasal Septum, Sinusitis and related disorders, Surgery on tonsils/Adenoids, gastric and duodenal ulcer, any type of Cysts/Nodules/Polyps, and any type of Breast lumps, Hypertension and Diabetes and related complications during the first two years (24 months) of continuous operation of this insurance cover. Diabetes & Related complications include: Diabetic Retinopathy, Diabetic Nephropathy, Diabetic Foot/Wound, Diabetic Angiopathy, Diabetic Neuropathy, Hyper/Hypoglycemic Shocks. Hypertension & Related complications include: Coronary Artery Disease, Cerebrovascular Accident, Hypertensive Nephropathy, Internal Bleed/Haemorrhages. If these diseases are pre-existing at the time of proposal or subsequently found to be pre-existing exclusion 1 above shall apply. • Any treatment arising from or traceable to pregnancy, childbirth including caesarean section until 48 months of continuous coverage has elapsed since the inception of the first policy with us. However, this exclusion/waiting period will not apply to Ectopic Pregnancy proved by diagnostic means and certified to be life threatening by the attending Medical Practitioner. • Any Domiciliary Hospitalisation/Treatment. • Circumcision unless necessary for treatment of a disease, illness or injury not excluded hereunder or due to an accident. • Genetic disorder and stem cell implantation/surgery. • Dental treatment or surgery of any kind unless necessitated due to an accident and requiring minimum 24 hours hospitalisation or treatment of irreversible bone disease involving the jaw which cannot be treated in any other way, but not if it is related to gum disease or tooth disease or damage. • Birth control procedures, hormone replacement therapy and voluntary termination of pregnancy during the first 12 weeks from the date of conception. • Routine medical, eye and ear examinations, cost of spectacles, laser surgery for cosmetic purposes or corrective surgeries, contact lenses or hearing aids, vaccinations except post-bite treatment or for new born baby up to 90 days, issue of medical certificates and examinations as to suitability for employment or travel. • All expenses arising out of any condition directly or indirectly caused due to or associated with human T-call Lymph tropic virus type III (HTLV-III) or Lymphadinopathy Associated Virus (LAV) or Acquired Immune Deficiency Syndrome (AIDS), AIDS related complex syndrome (ARCS) and all diseases/illness/injury caused by and/or related to HIV and sexually transmitted diseases. • Vitamins and tonics unless forming part of treatment for disease, illness or injury and prescribed by a Medical Practitioner. • Instrument used in treatment of Sleep Apnea Syndrome (C.P.A.P.) and Continuous Peritoneal Ambulatory Dialysis (C.P.A.D.) and Oxygen Concentrator for Bronchial Asthmatic Condition, Infusion pump or any other external devices used during or after treatment. • Artificial life maintenance, including life support machine use, where such treatment will not result in recovery or restoration of the previous state of health. • Treatment for developmental problems including learning difficulties eg. Dyslexia, behavioural problems including Attention Deficit Hyperactivity Disorder (ADHD). • Treatment for general debility, ageing, convalescence, run-down condition or rest cure, congenital external anomalies or defects, sterility, infertility including IVF, impotency, venereal disease, puberty, menopause or intentional self-injury, suicide or attempted suicide (whether sane or insane). • Certification/Diagnosis/Treatment by a family member or from persons not registered as Medical Practitioners under the respective Medical Councils, or any diagnosis or treatment that is not scientifically recognised or experimental or unproven. • Ailment requiring treatment due to use, abuse or a consequence of an abuse of any substance, intoxicant, drug, alcohol or hallucinogen and treatment for de-addiction, or rehabilitation. • Any illness or hospitalisation arising or resulting from the Insured/Insured person or any of his family members committing any breach of law with criminal intent. • Any treatment received in convalescent homes, convalescent hospitals, health hydros, nature cure clinics or similar establishments. • Prostheses, corrective devices and medical appliances, which are not required intra-operatively for the disease/illness/injury for which the Insured/Insured Person was hospitalised. • Any stay in Hospital without undertaking any treatment or any other purpose other than for receiving eligible treatment of a type that normally requires a stay in the hospital. • Treatment of any mental illness or sickness or disease including a psychiatric condition, disorganisation of personality or mind, or emotions or behaviour, Parkinson's or Alzheimer’s disease even if caused or aggravated by or related to an Accident or Illness or general debility or exhaustion (“run-down condition”). • Any cosmetic surgery unless forming part of treatment for cancer or burns, surgery for sex change or treatment of obesity/morbid obesity or treatment/surgery/complications/illness arising as a consequence thereof. • Charges incurred primarily for diagnostic, X-ray or laboratory examinations or other diagnostic studies not consistent with or incidental to the diagnosis and treatment even if the same requires confinement at a Hospital/Nursing Home. • Costs of donor screening. • Any form of Non-Allopathic treatment, Naturopathy, Hydrotherapy, Ayurvedic, Homeopathy, Acupuncture, Reflexology, Chiropractic treatment or any other form of indigenous system of medicine. • Insured/Insured Persons whilst engaging in speed contest or racing of any kind (other than on foot), bungee jumping, parasailing, ballooning, parachuting, skydiving, paragliding, hang gliding, mountain or rock climbing necessitating the use of guides or ropes, potholing, abseiling, deep sea diving using hard helmet and breathing apparatus, polo, snow and ice sports or involving a naval military or air force operation. • Insured/Insured Person whilst flying or taking part in aerial activities (including cabin crew) except as a fare-paying passenger in a regular Scheduled Airline or Air Charter Company. • All expenses caused by ionising radiation or contamination by radioactivity from any nuclear fuel or from any nuclear waste from the combustion of nuclear fuel. • All expenses directly or indirectly, caused by or arising from or attributable to foreign invasion, act of foreign enemies, hostilities, warlike operations (whether war be declared or not or while performing duties in the armed forces of any country), civil war, public defense, rebellion, revolution, insurrection, military or usurped power. • All non-medical expenses including but not limited to convenience items for personal comfort not consistent with or incidental to the diagnosis and treatment of the disease/illness/injury for which the Insured/Insured Person was hospitalised, Ambulatory devices, walker, crutches, belts, collars, splints, slings, braces, stockings of any kind, diabetic footwear, glucometer/thermometer and any medical equipment that is subsequently used at home except when they form part of Room expenses. • Service charges levied by the hospital, except registration/admission charges. What is the cancellation procedure for my:health Medisure Prime? In case you wish to cancel the Policy, you may intimate us by giving 15 days notice in writing and we will refund the premium for the unexpired term as per the short period scale given below. Period of Cover up to Refund of Annual Premium rate (%) 1 Month 75% 3 Months 50% 6 Months 25% Exceeding Six Months up to 365 days NIL For two year policies we will retain 15% of the premium for the balance period. Eg. 2 Year Policy issued for 730 days. Cancellation request received on day 395(1 year and 1 month). The amount refunded will be calculated as follows: The amount to be refunded will be 15% less than the pro-rata premium for the balance period. 2 year premium `1000. Utilised period 395 days, unutilised period 335. Pro-rata premium for unutilised premium will be `458.9 Refund amount shall be `458.9-15% i.e (`458.9 – `68.83) = `390 No refund will be done if you have reported a claim under the policy. What are the renewal terms of my:health Medisure Prime? You can renew your Policy by making a request to us before the expiry date. You can also renew policy within 30 days from expiry of your Policy subject to the fact that the premium for the renewal is received within the same period. In such a situation, you will be eligible to ‘continuity benefits’ like the cumulative benefits and others. Please note that we shall not be liable for any claims arising out of ailments/hospitalisation during the period between expiry and renewal. If the renewal of the Policy is not done within 30 days of due date, it will be considered as a new policy. We will not be liable to pay hospitalisation expenses incurred during break period. Any disease/condition contracted in the break in period will not be covered and will be treated as Pre-existing condition. The Policy can be renewed for lifetime unless: I. Any fraud, non cooperation, misrepresentation or suppression of material facts as sought to be declared on the Proposal form by You or on Your behalf is found either in obtaining insurance or subsequently in relation thereto or, II. We have discontinued issuance of Policy under this Product, in which event You will have the option of renewal under any similar Policy being issued by us; provided however, benefits payable shall be subject to the terms contained in such other Policy. Such modification or revision of the terms and conditions of the Product shall be intimated to you 3 months in advance along with reasons of modification and revision. A change in the Sum Insured is allowed on renewals, however it may be subject to Medical test and subsequent Underwriting approval. Any enhanced Sum Insured during subsequent policy renewals will not be available for an illness, disease, injury already contracted under the preceding policy periods. All Waiting periods as defined in the Policy shall apply for this enhanced limit from the effective date of enhancement of such Sum Insured considering such Policy Period as the first Policy with Us. Sum Insured enhancement will be subject to Underwriting approval. Based on the experience of the Product, Premium, terms and conditions may be revised subject to prior approval of Insurance Regulatory and Development Authority. Such revision shall be intimated to you 3 months in advance with an option of renewal under any similar Policy being issued by us. However, benefits payable shall be subject to the terms contained in such other Policy. Individual Claims experience loading is not applicable under the Policy. If you are above the age of 70 years, there is a 25% co-payment obligation as there is no further premium increase. For any changes desired at the time of Renewal, please communicate with us. In case of floater Policies, where dependent child crosses age 18 years, renewal can be done in a separate Policy under the same Product or any other available Products with continuity benefits. The Company reserves its right to vary the premium from time to time subject to approval of IRDA (Insurance Regulatory and Development Authority). Portability Option: Portability will be provided on this Policy in accordance to IRDA guidelines issued on Portability from time to time. You may approach another insurer at least 45 days before the expiry date to avoid any break in coverage. Portability benefits are not automatically applicable under the Policy unless application for portability has been specifically made and subsequently accepted by the Company. How are the Zones classified for premium calculation? For the purpose of premium calculation, there are 3 major Zones classified on the basis of your location: Zone I (All India Cover): Mumbai, Thane, Navi Mumbai, Delhi and NCR Regions Zone II: Chennai, Hyderabad, Bangalore, Pune, Kolkata and Gujarat Regions Zone III: Rest of India (other than Zone I and Zone II locations) • Zone I premium gives you an All-India cover. i.e., you can avail treatment all over India without any co-payment • Zone II premium allows you to avail treatment in Zone II and Zone III locations without any sub-limits. But if you have to avail treatment in a Zone I city, you will have to bear a 10% co-payment on each and every claim • Zone III premium allows you to avail treatment only in Zone III locations without sub-limits. In case you have to avail treatment in a Zone II location, you will have to bear a 10% co-payment on each and every claim. And if you have to avail treatment in a Zone I location, you will have to bear a 20% co-payment on each and every claim You have a choice to opt for Zone I (All India Cover) even if you belong to a Zone II or III by paying the premium applicable under Zone I. The Premium will be based on the age of the senior most member in the Policy, Zone of cover, total number of persons you wish to cover, the policy tenure and Sum Insured you opt for. How do I get my:health Medisure Prime Policy? For person below 45 years with a clean medical history, all you need to do is fill in the necessary details in the Proposal Form and hand it over along with your premium payment instrument to your Insurance Advisor or at any of our offices. You will be given an Instant Policy Kit. All proposals above 45 years or for proposals below 45 years of age with an adverse medical/claim history will have to undergo a pre-enrolment medical test. What is the process to lodge a Claim under my:health Medisure Prime? For cashless* hospitalisation: (*cashless facility is available only at our network hospitals) • You will need to contact us at the contact numbers provided on your health card immediately • You need to submit a Cashless Request form to us, along with all information and documents pertaining to the illness as required • Our representative will guide you further and help you process the request and based on the coverage under your policy the cashless request will be approved. For a reimbursement of expenses: (at other hospitals outside of our network) or at network hospitals where cashless is not availed: • You will need to collate the original bills along with other documents as required and submit it to the address mentioned in your policy schedule. In case of planned hospitalisation, please inform us 72 hours in advance and in case of emergency within 24 hours of admission.
