Family Information Form This form is for use in helping with the parent/sitter hand-off and is intended to be used multiple times -- for each new sitter you meet! Not all questions will apply to your family, just complete the ones that do and add anything that you feel we missed! This form is not meant to bring stress or worry, please only add info you feel comfortable sharing, and have fun with it!

Other Helpful Information: This info will not apply to every home. Please just do your best to be complete and thorough, and skip what doesn’t apply! NAPS AND BEDTIME: Please indicate anything your sitter should know to make nap/bedtime go smoothly: For example: Are there any items your child NEEDS in order to fall asleep (Blankets, pacifier, white noise)? What are the important parts of your routine? How does your baby monitor work? Teeth brushing? Pull-Ups/overnight diapers? Nightlights? Extra Jammies? Etc.
ELECTRONICS: Please indicate rules and guidelines surrounding screen time/electronics use in your home: For example: Is TV time allowed? How much? What show/movie ratings are appropriate? Are video games/iPad use allowed? Time limit? If the child has their own phone, what rules are in place?
HOUSEHOLD RULES: Please indicate what rules are expected to be followed and how you would like the sitter to handle a situation should any of these rules be broken/followed: For example: Do the children clear the table after eating? Do the children tidy their own play area? Any behaviors strictly NOT tolerated? Does your family do time-outs? Any privileges to be lost? Are there any incentives for good behavior? Sticker/chore chart? Any specific words/phrases when correcting or praising a behavior?
