Fort Wayne Obedience Training Club, Inc. Public Class Application Dear Dog Owner: Your desire to enroll your dog in our next obedience training class has been brought to my attention. *See application form for dates and times, as these may vary from year to year. Attached is an application for the class beginning on * . The class will be held at the Allen County Fairgrounds in the Home Loan Bank Building. The classes are one hour long and begin at * for eight consecutive weeks. Since the class is limited in size, you must fill out the attached form and return it to the address below seven (7) days prior to the first class. Please note that this does not guarantee you a spot in a class, as it is a first come, first serve basis.
Your dog must be: • Very close to 6 months of age OR older. • Inoculated against Distemper, Hepatitis and Leptospirosis (DHL), Parvo Virus, and other immunizations recommended by your veterinarian. Rabies vaccination is required by law. You may either send a copy of the receipt or computer read-out showing proof of this with your application, or bring it with you on the first night of class. If you send an original, it will be returned to you. You, as the handler, must: • Be at least 12 years of age and physically capable of handling this dog. • Not feed the dog his usual evening meal until after class. • Be comfortably dressed with low-heeled tie-on shoes. • Exercise the dog immediately before coming into class, giving him/her a chance to relieve themselves. (Be sure to clean up after your own dog!) The following items are required: • A properly fitted collar and a leather or woven leash (6 feet long), both of which will be available from the club at very reasonable prices. • The receipt from the veterinarian showing proof that the dog has had all the necessary immunizations. YOU WIL NOT BE PERMITTED TO WORK YOUR DOG IN THE CLASS IF YOU HAVE NOT SHOWN THIS PROOF.
PLEASE PLAN TO ARRIVE 15 MINUTES PRIOR TO YOUR CLASS THE FIRST NIGHT OF CLASS! Also, DO NOT bring your dog to the FIRST night of class. ***The fee for the entire 8 week course is $80.00 and must accompany the application so that a place may be reserved for you. In consideration of the acceptance of this application and upon entering this dog in the class, I hereby agree to hold the Fort Wayne Obedience Training Club, Inc., its members, trainers, officers and all persons connected with the club HARMLESS from: (1) any loss or injury which may occur to any person or thing by biting, or any act of my dog while in or upon the premises or grounds while being delivered or removed or otherwise handled, and (2) the disappearance and/or loss by theft of otherwise, and/or death of the above named dog. I personally assume full responsibility and liability for my dog’s actions, for all damages and injuries caused by the negligence or carelessness of the said Club, its members or any person or persons and/or by any other cause directly or indirectly operating while my dog, members of my family, or I am on the Club premises.
