~~There have been changes made, please read this page thoroughly~~ 1. Fees: The fee structure for Langenburg Minor Athletics Association is comprised of two parts: the Saskatchewan Hockey Association (SHA) fee and the Langenburg Minor Athletics Association fees. The fees are based on components such as ice time fees, equipment fees, referee charges, etc. For your convenience we have incorporated one registration form per child which includes the fees for LMAA and the Rink Board Skating fees (LCDB). 2. Registration Form: Please fill in all the information requested on the Registration Form. The registration form is specific to the 2015-2016 hockey season. Read all options available and correctly “tick” your choice. Cheques are to be dated no later than September 10, 2015. You will submit THREE cheques: *One payable to LMAA dated for September 10, 2015 for all applicable hockey and rink fees, gate pass and optional hockey camp, *one Payable to LMAA and post-dated for March 31, 2016 for jersey deposit and *one payable to LCDB and post-dated for March 31, 2016 for Rink Kitchen fee. 3. Can Skate Only Families: There is a separate form now for children who are in can skate only. You will submit TWO cheques: *One payable to LMAA dated for September 10, 2015 for can skate and applicable rink fee and *one payable to LCDB and post-dated for March 31, 2016 for Rink Kitchen fee. 4. Parental Conduct Agreement: Please read carefully the Langenburg Minor Athletics Association Parental Conduct Agreement on the last page of the registration form (that page is yours to keep). Both parents/guardians (players 18 & older sign on their own behalf) must sign the Registration Form acknowledging concurrence of the Parental Conduct Agreement before their child is allowed to play hockey in the Langenburg Minor Athletics Association. 5. Alliance Elite Hockey Skills & Development Camp: This optional hockey skills camp will commence Sept 21, 2015. Each session will be 1 hour on ice and 30 minutes dryland. The first week will run Sept 21-25th, second week Sept 28-Oct 2nd each for a $150 fee. If you choose to attend both the cost will be $275. The instructors have over 27 years Pro experience in the industry. (a brochure will be out soon) Tentative start times are: Pre-Novice & Novice 5:00pm, Atom & Pee Wee 6:15pm, Bantam & Midget 7:30pm 6. Mark your calendar for some 3 on 3 tournament action starting Oct 5th – Oct 19th! More info to follow in the next few weeks.

Registering the above mentioned player provides LMAA the ability to share name, birth date, address, postal code, phone number and hockey associated information with other hockey and sports organizations at the discretion of the Langenburg Minor Hockey Association.

LANGENBURG MINOR ATHLETIC ASSOC. / PARENTAL/GUARDIAN CONDUCT AGREEMENT 2015-2016 SEASON The Parental/Guardian Conduct Agreement is designed to help parents/guardians understand their role with their son’s/daughter’s participation in hockey. The Player Registration Form must be signed by the parent(s) or guardian(s) acknowledging concurrence of the Parental/Guardian Conduct Agreement before their child is allowed to play hockey in Langenburg Minor Athletics Association (LMAA). If the document has not been signed the player will NOT be registered. Parental/Guardian Conduct: I will not force my child to participate in hockey. I will remember that my child plays hockey for his or her enjoyment, not mine. I will encourage my child to play by the rules and to resolve conflict without resorting to hostility or violence. I will teach my child that doing one’s best is as important as winning so that my child will never feel defeated by the outcome of the game. I will make my child feel like a winner every time by offering praise for competing fairly and hard. I will not ridicule or yell in a derogatory fashion at any player on either team, at game officials, team officials or any parent. I will remember that children learn by example. I will applaud good plays by both my child’s team and their opponents. I will never question the game official’s judgment or honesty in public or in front of my child. I will remember that officials are developing their skills similar to the players they are officiating. I will support LMAA’s efforts to remove verbal and physical abuse from hockey games. I will respect and show appreciation for the efforts of coaching staff and team officials who volunteer their time for hockey for my child. I will act responsibly towards hockey facilities and with those involved in the game – players, coaches, officials, spectators and facilities staff. I will abide by my team’s communications rules between coaches and parent’s (i.e. through the parent liaison). I will observe a 24-hour waiting period before contacting coaches, team or league officials after any incident. I will be responsible for my guests at my child’s hockey games, encouraging them to respect and abide by these same rules. I understand that Langenburg Minor Athletics Association will apply restrictions of privilege if I do not adhere to the Parental/Guardian Conduct Agreement and LMAA’s Rules & Regulations. These restrictions will include spectator privileges to player suspension based on frequency and severity of incidents. Teams coaches/managers are responsible to notify the Division Director immediately of any parental/guardian conduct offenses.
