2310 Skokie Valley Rd. Highland Park, Il 847-432-5055 www.froggponddive.com PADI Open Water Scuba Certification Before you start, some basics you should know: Students will need to pass a swim test, consisting of a 200 yard swim and a 10 minute tread/float. A Medical Statement is required. Depending on the answers to this questionnaire, a student may need to have a doctor’s sign-off before beginning training in the pool. A set of release forms is required. For minors, parents must also read and sign the forms. Students as young as 10 years old can become certified divers. There are 3 parts to earning your certification: 1. 2. 3. Classroom – Actual “book” knowledge is the first of the three steps towards an Open Water Diver certification. a. We use the PADI eLearning classroom option rather than a traditional classroom, so the student can learn at the own pace on their own schedule. b. Just use the PADI eLearning link on our home page at www.froggponddive.com to get started. c. Once the online course is 100% complete the student must also schedule time for a 25 question quiz with an instructor at the shop. d. Cost (paid to PADI) is $146. Pool sessions – In-water skills are first developed in a pool, usually over 3 weekly sessions. Check with the shop or visit the on-line calendar for actual dates. a. Students must have the Introduction and Section One (including quiz/assessment) of the eLearning class done prior to their first in-water training date. b. All paperwork, including medical forms, must be complete prior to first pool date. c. Students need to have their own mask, snorkel, and fins for use in the pool. Registered and paid students receive a 10% discount for these items at the shop. All other equipment (including wetsuit) is provided by the shop. d. Students must bring a swim suit and a towel to each pool session. e. Cost is $159 for scheduled classes, private class arrangements can be made at an additional cost. A $56.80 package of supplemental materials for the eLearning class is also a required purchase at the time of class registration. Open Water Dives – Four “checkout” dives across two days are required to complete your certification. a. All pool work and eLearning must be completed prior to these dives. b. Dives must be completed within 1 year of completing the pool sessions. This can be extended by taking a pool “refresher” course. c. These can be done locally with the Frogg Pond Dive shop on scheduled weekends. The local season runs April through November, but the dives are best scheduled from June to October. Cost of $299 includes all training, certification card, gear rental, tanks and air fills. A $20/day quarry entrance fee is also required upon arrival. d. Dives can also be done on a warm water trip, either with a Frogg Pond shop trip where available, or as a referral on your own vacation destination. All prices current as of 1/1/2014. Subject to change.
