Add a Segment 1 Discontinue a Segment 2 Add Segment Scenario: In circumstances where an additional segment is required, such as recording a PreOp delay, an appropriate segment can be added. Mini Manual Note: Theatres’ Staff 1. Add a Segment 2. Discontinue a Segment 3. Activate a Segment 4. Terminating documents Ensure you are in the right documentation for adding the segment, i.e.. PreOp Doc or IntraOp Doc. Discontinue a segment Scenario: Diathermy is not relevant to some cases such as an eye procedure, therefore the segment needs to be discontinued. Within IntraOp Docs of the patient record Step 1. Right click Diathermy segment. Step 2. Select Discontinue Diathermy RUH segment. Within a patient record Step 1. Right click on any segment. The Discontinue window opens. Step 2. Select Add Segment. Step 3. Select Segment Not Applicable to Case from the drop-down. Step 4. Click OK to close the window. Result: The Diathermy RUH segment is discontinued and greyed out. The Add Segment window opens. Step 3. Select the required segment from the left hand side Available pane. Step 4. Click on the right facing single chevron to move the selection to the Selected pane. Step 5. Click OK to close the Add Segment window. Support available through the following options: 1. Deactivated segments are greyed out. Business Rules Terminating documents Business Rules Scenario: Terminating documents can be used in various situations such as abandoning cases and PACU only situations With an open perioperative window Caution: Terminating doc’s is a permanent feature, once completed it cannot be undone. Step 1. Step 2. Result: Right click on the greyed out segment to reactivate. Select Activate , in this example Activate Diathermy RUH. Step 1. Using Surginet select the RUH Theatres tab. Step 2. Double click on the required patient. The Document Selection dialog opens. The reactivated segment can be completed. Step 3. Select the relevent document (PreOp, IntraOp or Post Op) to terminate Step 4. Click OK button. The Patient Record opens the Perioperative Doc requested above documents listed. Step 5. Select Document > Terminate from the Toolbar menu. The Terminate Document dialog opens. Step 6. Select Document Not Applicable to Case. Step 7. Click OK to close the dialog and return to the Perioperative Doc window. The Confirm Termination dialog opens. Step 8. Click Yes to terminate the document. The fields within the segments grey out and cannot be completed. The Doc has been terminated. Step 9. 1. All surgical trays, supplementaries and prepacks should be recorded on the Instrument Tray Details segment on the Intra Op Document using a hand held barcode scanner. 2. All Loan Equipment should be recorded on the Loan Equipment Details Segment on the Intra Op Document. 3. The Op Note must be written up using PowerChart/Millennium before the Patient leaves the Theatre Complex, and it must record a minimum of “Actual Procedure” and “Post Op Instructions”. 4. The Op Note must be printed and attached to the Patient Notes. 5. The PreOp, IntraOp and PostOp Summaries must be printed, and a copy of each attached to the Patient Notes, and a physical signature on each paper document is mandatory. 6. The WHO Checklist must be completed for each case. 7. All People present in Theatre must be recorded. 8. Timings should be documented accurately. 9. If a Surginet patient record is locked by another member of Staff, it should only be unlocked with agreement from the lock holder. 10. The preloading of patient data onto Surginet should only happen while the patient is in the Anesthetic Room and only when the procedure is definitely going ahead. 11. Perioperative Documents must be finalised. 12. Every Patient episode in theatre must be documented in Surginet. If more than one doc needs to be terminated repeat the above steps. Step 10. Complete the relevent docs. Step 11. Finalise and print. Result: Page 4 The selected docs have been terminated and the remaining documents have been completed, finalised and printed. Page 5 Page 6
